Newly Appointed Postdoctoral Researchers/Visiting Scholars
All Postdocs and Visiting Scholars
must complete a new hire enrollment form in order to be enrolled in these plans. You may accomplish the following by completing an enrollment form:
- Select the benefits in which you wish to enroll
- Select the benefits for your eligible dependents
Waive coverage for yourself and/or eligible dependents
Complete a Beneficiary Form for the Life and AD&D Insurance
Acknowledge that you have read the General Notice of COBRA Continuation Rights
Coverage for benefits begin on your Job Begin Date displayed in Section 1 of the enrollment form. Regardless of any administrative delays in the process of finalizing your employment record, your enrollment will be retroactive back to your appointment start date. All benefits are billed one month in advance. This means that deductions taken from your paycheck at the end of the current month are to pay for next month’s coverage. For example, if your job begin date is in October and you enroll in November your November paycheck will have deductions for October, November and December.
Waiving Coverage for Yourself and/or Your Dependents
If you do not wish to enroll in the PRBP, you may waive (decline) this coverage. Please complete the appropriate sections of the New Hire Enrollment Form in order to waive the coverage. If you waive coverage for yourself and/or your eligible dependents, you will not be eligible for coverage until the following Plan Year, unless you experience a qualifying event, such as marriage, birth or adoption of a child or a loss of coverage. If you are a Visiting Scholar holding J1 Visa status and wish to waive this coverage, you must furnish proof that your medical coverage meets the J1 Visa requirements. Postdocs Working Abroad
If you are a postdoc working outside the United States none of the medical or dental plans associated with the PRBP are available for enrollment. You are automatically enrolled in the Life/AD&D insurance and do have the opportunity to choose the Voluntary Life Option. For information on travel policies, you may consult an insurance carrier directly, a travel insurance broker in your area, or the U.S. Department of State’s website at http://travel.state.gov/content/passports/en/go/health/insurance-providers.html.

Please follow the links on the side of this page to assist you in learning about the plans and completing the New Hire Enrollment Form. In order to access the form, you will use your Chicago ID Number and Date of Birth for your initial login. If you are unsure of your Chicago ID number, please click on this link to access your number https://cnet.uchicago.edu/ams/servlet/ChicagoIDSearch. Once you login, the system will ask you to create your unique password. Please do not use your CNET password. Your CNET password must be kept solely for the use of the University of Chicago site . Please create a separate, unique password for this site.
The New Hire Enrollment Form is a secure online enrollment tool allowing you to make all of your benefit selections for all plans, or to waive coverage. This form may be printed once you have completed it properly. In order to streamline your enrollment process, we recommend that you first print the New Hire Enrollment Form Instructions.
