Enrolling in this benefit program is simple. Before you start, please gather the following information:

1. Your Vanderbilt University email address
2. If enrolling in the Aetna DHMO Dental Plan, you will need the office number of your Primary Care Dentist to complete the enrollment form (please visit the Find a Provider page to locate the office number)
3. Your eligible enrolling dependents' dates of birth
