New Postdoc Enrollment

Enrolling in this benefit program is simple. Before you start, please gather the following information:

1. Your University of Chicago ID number. If you do not know your University Chicago ID, please use the following link for access to the directory: https://cnet.uchicago.edu/ams/servlet/ChicagoIDSearch
2. If enrolling in the Aetna HMO Medical Plan, you will need the office number of your Primary Care Physician to complete the enrollment form (please visit the Find a Provider page to locate the office number)
3. Your e-mail address
4. The name of your department contact
5. Your department contact's email address
6. Your eligible enrolling dependents' dates of birth

Coverage Begins:

Coverage for benefits begin on your Job Begin Date displayed in Section 1 of the enrollment form. Regardless of any administrative delays in the process of finalizing your employment record, your enrollment will be retroactive back to your appointment start date. All benefits are billed one month in advance. This means that deductions taken from your paycheck at the end of the current month are to pay for the next month's coverage. For example, if your job begin date is in October and you enroll in November, your November paycheck will have deductions for October, November and December.
