Enrolling in this benefit program is simple. Before you start, please gather the following information:

1. Your City of Hope ID number
2. If enrolling in the Aetna HMO Medical Plan, you will need the office number of your Primary Care Physician to complete the enrollment form (please visit the Find a Provider page to locate the office number)
3. If enrolling in the Aetna DHMO Dental Plan, you will need the office number of your Primary Care Dentist to complete the enrollment form (please visit the Find a Provider page to locate the office number)
4. Your eligible enrolling dependents' dates of birth

Please Note: If you are having trouble creating an account using your City of Hope ID, we may not have received your HR information. If the problem persists please contact GBS.

Once the form is completed, signed and notorized, please scan and email the PDF copy to Gallagher Benefit Services (GBS) at: UniversityServices.GBS.cohbp@ajg.com. Once Gallagher Benefit Services receives the document, we will proceed with your enrollment into the plans.
