IMPORTANT ENROLLMENT INFORMATION
You may enroll in a plan at any time, however, your enrollment is not official until your first payment is received.
You may add a dependent at any time.
Your coverage should begin on the first date of your entry into the United States
(please indicate this date on your enrollment form) or the date your first invoice is paid, whichever is latest.
Enrolling and/or making your payment after the date you enter the United States will result in your coverage
not taking effect until that payment date.
You have the option to pay monthly, quarterly, or all at once (no longer than 12 months at a time).
Once your invoice is paid, you will receive instructions on how to download your Medical ID, Visa Letter,
and Declaration Letter around 5-7 business days later

Important Information
Eligibility:
As a newly appointed Visiting Scholar/Researcher, you are required to be enrolled in an insurance plan that meets the Department of State J-1 Visa requirements beginning the date you enter the United States, up to the date you leave the United States. If enrolling, please indicate your entry and exit dates on the enrollment form to ensure proper coverage.
Please submit your enrollment, or waiver if you have alternate coverage, at your earliest convenience to avoid being marked as out of compliance with the University and Department of State.
This enrollment and waiver form are only intended for use by international visiting scholars. U.S. Citizens and U.S. Permanent Residents have different requirements; please refer to your school for those requirements and your options for health insurance.

J Visa Scholars
International Visiting Scholars have the option to enroll in a Dental or Vision plan for an additional charge. Note these plans are not managed by Gallagher Benefits; please reach out to the respective companies with questions.
J Visa Scholars have the option to enroll in the Dental Plan that offers a breadth of coverage through a PPO model.
J Visa Scholars are subject to the U.S. State Department J-1 Visa requirements and any additional requirements imposed by the University.
Information For all J Visa Scholars
All J Visa Scholars must meet the insurance policy requirements listed below:
- $100,000 per accident & illness in medical coverage
- A deductible not to exceed $500 per accident or illness
- Coinsurance that does not exceed 25%
- Medical evacuation coverage must be at least $50,000
- Repatriation coverage must be at least $25,000
- Carrier must be at least A- rated or backed by the full faith and credit of the Exchange Visitor's government
- For scholars residing in the United States for more than 12 months, pre-existing conditions must be a covered benefit, with the waiting period for coverage no more than 12 months. If the insurance policy has the pre-existing conditions benefit as a set amount, benefit coverage must be the same as illness coverage.
