• Eligibility for Enrollment: Information is provided to assist you in understanding the eligibility criteria: the time frame for making your plan choices and your family member eligibility.
• New Hire Enrollment Form Instructions: This document provides step-by-step instructions for each section of the form. Please print the instructions first to assist you in completing the form.
• Enrollment Form Login: When you click on this link you will be taken to the Enrollment Form Setup page. This is where you will create your account. If you are a returning user, simply use your Vanderbilt email address and your previously created personal password. Lost password assistance is available.
• Medical PPO Buy-Up Costs: This page will explain the contributions toward the buy-up to the Aetna PPO 90/70 plan. It also provides information pertinent to paying for the plan and the ongoing billing procedure.
• Insurance Carrier Privacy Notice: Please read this notice to learn how the insurance carriers offering coverage for the PTBP maintain your privacy.
