Aetna Dental HMO Plan

The Aetna Dental HMO Plan is a comprehensive dental plan offering a breadth of coverage with no annual deductible or dollar maximums. Differing from the PPO plan described below, you will choose a Primary Care Dentist through the online provider directory and see that dentist for all of your care. There is not an out-of-network benefit with this plan. Should you require a specialist, your Primary Care Dentist will provide the referral to the appropriate specialist. You may choose a separate Primary Care Dentist for each member of your family. For your children 7 years of age or older, please choose a Pediatric Dentist as their Primary Care Dentist.

Offering a broad range of coverage, this plan provides most preventive procedures covered at 100%, with no copay required. Such preventive services in this category are periodic oral exams and cleanings (available two times per calendar year). please click on Aetna Dental HMO Plan Benefits.

Aetna Dental PPO Plan

The Aetna Dental PPO Plan provides you 100% coverage for your preventive services, as oral exams, routine cleanings and x-rays. This plan offers you the flexibility of seeing a dentist either in-network by choosing a dentist in the provider directory, or you may see a dentist of your choice out-of-network. Choosing a dentist within the Aetna provider directory will allow you to pay lower costs than if you seek services from a dentist not listed in the directory. There are over 115 in-network general dentists to choose from within a five mile radius of the campus, not including specialists such as endodontists and periodontists.

If you are choosing the HMO dental plan, please provide the office number of your dentist choice in the appropriate space in Section 6 of the enrollment form.
