Dental Provider Directory Information

For Aetna Dental HMO Enrollees

The Aetna Dental HMO Plan offers a wide selection of dentists from which to choose. You may search by Primary Care Dentist, specialists or the combination of the two. In order to view the directory and select your Primary Care Dentist, please follow the instructions below. You may wish to print this page first, then follow the instructions:

• Click here for the Aetna DocFind Directory.
• Scroll down to Geographic Information and select a Search Type, either zip code, city or county. We recommend you use your home zip code to narrow your search.
• Enter the Distance in Miles to read no more than 30 miles, a requirement by Aetna DHMO.
• Select Primary Care Dentists.
• Under DMO®/Managed Dental, select Dental Maintenance Organization (DMO).
• At this point, you may either narrow your search by the criteria provided, or view a list of dentists. The dentists will be listed in groups of 50, in alphabetical order.
• Please enter the Primary Care Dentist’s office number in the appropriate space on the enrollment form in Section 6.

For Aetna PPO Dental Enrollees

When you select the Aetna Dental PPO Plan, it is not necessary to choose a Primary Care Dentist. You may choose from a general list of dentists, or by specialty. In order to view a list of dentists in your area, please follow the directions below. You may wish to print this section first to assist you in navigating the site:

• Click here for the Aetna DocFind Directory.
• Scroll down to Geographic Information and select zip code, city or county. We recommend you use your home zip code to narrow your search.
• Enter the Distance in Miles.
• Select Dental Providers.
• Select All Dental Providers or Specialists.
• Select Dental PPO/PDN with PPO II Network.

At this point, you may either narrow your search by the criteria provided, or view a list of dentists. The dentists will be listed in groups of 50, in alphabetical order.
