Enrollment Form Login
If you have previously performed the login process, you will log in below as a returning user with your Vanderbilt email address and your previously created personal password.
If you have not visited the enrollment form previously, you will be required to create a login to perform the initial enrollment login process. Please keep your personal password in a safe place for future reference if you need to login at a future date.
You will have 25 minutes to make your benefit selections and complete the sections regarding whom you are enrolling (yourself and your eligible dependents). Please have the following information ready before you login to avoid your session timing out prior to completing the form: 1) Enrollment Form Instructions or Open Enrollment Form Instructions; 2) the office number of your Primary Care Dentist for if you are enrolling in the Aetna DHMO dental plan, 3) Your eligible enrolling dependents' dates of birth.
